FLORIDA DEPARTMENT OF HEALTH IN SANTA ROSA COUNTY JAY LOCATION

Beginning July 1, the Florida Department of Health in Santa Rosa County will begin new  clinic hours at its Jay location at 14122 Alabama Street behind Jay Hospital.  Services will be offered from 9:00 a.m. to Noon the first Wednesday of the month.  Immunizations will be provided on a walk in basis.  Appointments will be required for other services.  The change in clinic hours is in response to a decline in demand for services and budget reductions.

"Clinic visits at the Jay location have declined to the point where it is no longer cost effective to offer services more than one morning a month. There will be additional clinics available for back to school services and flu vaccinations," said Sandra L. Park-O'Hara, A.R.N.P., administrator of the Florida Department of Health in Santa Rosa County.

Parents will still be able to obtain copies of their child's Florida Certificate of Immunization, commonly referred to as Form 680, which is required for school entry, during regular business hours, as long as their child's immunizations are up to date.  Appointments for other services can be made by calling the health department's Milton location at 850-983-5200, ext. 107. 

Immunizations are provided at no fee for eligible children through the age of 18 through the Vaccines for Children program.  There is a fee for adult vaccines.
